October 25, 2023 Opinion or Order Filing 5 ORDER Plaintiff Judvis Wallace filed his complaint using in part the court's pro se form titled Complaint for Violation of Civil Rights (Prisoner). Mr. Wallace also moved to proceed in forma pauperis using the court's form for incarcerated persons. Based on Mr. Wallace's address of record, it does not appear that he is currently incarcerated. The court therefore WILL DENY Mr. Wallace's motion WITHOUT PREJUDICE to his opportunity to renew the motion on the appropriate form. Similarly, the court DIRECTS Mr. Wallace to replead his complaint on the court's pro se general complaint form and not the court's form for incarcerated individuals. The court DIRECTS Mr. Wallace to file his amended complaint on or before November 15, 2023.
