Filing 14 ORDER (Service Packet) - IT IS ORDERED: Pursuant to Rule 25(d) of the Federal Rules of Civil Procedure, Director David Shinn is substituted for Charles Ryan with respect to only the official capacity claims in Count One. The knee injury claim in Count Four, and Counts Five and Six are dismissed without prejudice. Defendants Arizona Department of Corrections, Metanky, and Gordon are dismissed without prejudice. Defendant Ryan must answer the failure to intervene claim in Count One in his individual capacity; Defendant Shinn must answer the medical grievance policy claim in Count One in his official capacity; Defendant Corizon must answer Count Two; Defendant Igwe must answer Count Three; Defendant Hahn must answer the medication claim in Count Four, and Defendant Allen must answer Count Seven. The Clerk of Court must send Plaintiff a service packet including the First Amended Complaint (Doc. #13 ), this Order, and both summons and request for waiver forms for Defendants Shinn, Ryan, Corizon, Igwe, Hahn, and Allen. Plaintiff must complete and return the service packet to the Clerk of Court within 21 days of the date of filing of this Order.
