ORDER (Service Packet) - Plaintiff's Application to Proceed In Forma Pauperis (Doc. #6 ) is granted. Plaintiff must pay the $350.00 filing fee and is assessed an initial partial filing fee of $89.83. Defendants Unknown African American Doctor, Corizon, the official capacity claims against Defendant Thomas and the official capacity claims against Stewart in Counts I and II are dismissed without prejudice. Defendants Stewart and Thomas must answer the Complaint. The Clerk shall send Plaintiff a service packet including the Complaint (Doc. #1 ), this Order, and both summons and request for waiver forms for Defendants Stewart and Thomas. Plaintiff must complete and return the service packet to the Clerk of Court within 21 days of the date of filing of this Order. This matter is referred to Magistrate Judge Michelle H.
