Filing 3 ORDER - The Court construes the case initiating document (Doc. #1 ) as a motion for emergency injunctive relief ("Motion"). The Clerk of Court must substitute David Shinn for Charles Ryan and must add Centurion of Arizona, L.L.C., and Nurse Practitioner Karanja-Adams as party-Defendants. The Clerk of Court must immediately electronically send a copy of this Order to Sarah Barnes at slb@bowwlaw.com, and Michael Evan Gottfried at michael.gottfried@azag.gov. The United States Marshal must, immediately, personally serve copies of the Motion (Doc. #1 ) and this Order upon Defendants. Within 3 business days after personal service is effected, the United States Marshal must file a return of service for each Defendant. Within 15 days of service of the Plaintiff's Motion, Defendants must file a response. Within 30 days of the date this Order is filed, Plaintiff must either pay the $350.00 filing fee and $52.00 administrative fee or file a complete Application to Proceed In Forma Pauperis and a certified six-month trust account statement. If Plaintiff fails to either pay the $350.00 filing fee and $52.00 administrative fee or file a complete Application to Proceed In Forma Pauperis within 30 days, the Clerk of Court must enter a judgment of dismissal of this action without prejudice and without further notice to Plaintiff and deny any pending unrelated motions as moot. Within 15 days of the filing date of this Order, Plaintiff must file a complaint using the court-approved form. The Clerk of Court must mail Plaintiff the court-approved form for filing an Application to Proceed In Forma Pauperis (Non-Habeas) and a court-approved form for filing a civil rights complaint by a prisoner.
