Filing 4 ORDER (Service Packet) - Plaintiff's Application to Proceed In Forma Pauperis (Doc. #2 ) is granted. Count Two is dismissed without prejudice. Defendants Adel and Trusken are dismissed without prejudice. Defendants Shinn, Crabtree, Chavez, and Cruz must answer Count One of the Complaint. The Clerk shall send Plaintiff a service packet including the Complaint (Doc. #1 ), this Order, and both summons and request for waiver forms for Defendants Shinn, Crabtree, Chavez, and Cruz. Plaintiff must complete3 and return the service packet to the Clerk of Court within 21 days of the date of filing of this Order. This matter is referred to Magistrate Judge Michelle H.
