January 7, 2011 Opinion or Order Filing 8 ORDER granting 5 Plaintiff's MOTION/Application for Leave to Proceed in forma pauperis, Plaintiff must pay the $350.00 Filing Fee. Plaintiff is granted 60 days from the filing date of this Order in which to discover, by subpoena or otherw ise, the identity of each of the Doe Defendants and to file a "notice of substitution" providing each of the Defendant's names in place of their fictitious identities. The Clerk shall dismiss the Doe Defendants if Plaintiff fails to co mply. The Clerk must issue three subpoenas in blank and send them to Plaintiff. Defendant Wexford Health Solutions must answer Counts I and II. The Clerk must send Plaintiff a service packet including 1 Complaint, this Order, and both summons and r equest for waiver forms for Defendant Wexford Health Solutions. Plaintiff must complete and return the service packet to the Clerk within 21 days of the date of filing of this Order. This matter is referred to Magistrate Judge Mark E.
