Pay the $400.00 filing fee, or upon the filing of the amended complaint, file a Request to Proceed Without the Prepayment of Filing Fees with Declaration in Support, which includes a certified copy of Plaintiff's trust fund account statement for the 6-month period immediately preceding the filing of the complaint.
