(SEE ORDER FOR FURTHER DETAILS) The Court will afford Plaintiff a final opportunity to comply with the Court's October 29, 2018 Order. By December 28, 2018, Plaintiff must: 1. File a First Amended Complaint, using the Civil Rights Complaint Form attached to the Order, identifying all Defendants by name, stating the constitutional bases for the claim, and specifying when the alleged unlawful conduct occurred; and 2. Pay the $400.00 filing fee, or upon the filing of the amended complaint, file a Request to Proceed Without the Prepayment of Filing Fees with Declaration in Support, which includes a certified copy of Plaintiff's trust fund account statement for the 6-month period immediately preceding the filing of the complaint.
