ORDER GRANTING STIPULATION TO VOLUNTARILY REMAND ACTION TO STATE COURT AND DISMISS WITH PREJUDICE ALL CAUSES OF ACTION AGAINST PATIENTS L.S. AND S.W. by Judge Andre Birotte Jr.: Upon Stipulation #12 , IT IS HEREBY ORDERED that all causes of action pertaining to patients with initials L.S. and S.W., each of whom has plans governed by ERISA are hereby DISMISSED WITH PREJUDICE. This action shall be remanded to the jurisdiction of the Superior Court of California for the County of Ventura, Case Number 56-02020-00542353-CU-CO-VTA, for all purposes. Within ten days after the Ventura Superior Court receives the case on remand, Plaintiff shall file an amended complaint pertaining solely to the patient with initials R.B., and will not add any additional patients or causes of action to the case.
