MINUTE ORDER(In Chambers) ORDER RE: APPLICATION TO PROCEED IFP, Dkt. #2 by Magistrate Judge Margo A. Rocconi: On August 10, 2021, Plaintiff Marcel D. Ford ("Plaintiff"), proceeding pro se, constructively filed1 a Civil Rights Complaint pursuant to 42 U.S.C. 1983 ("section 1983"). ECF Docket No. ("Dkt.") #1 . Plaintiff also filed an Application to Proceed In Forma Pauperis ("IFP"). Dkt. #2 . The Court has reviewed Plaintiff's IFP Request. It does not, however, contain the required certificate of authorized officer. See Dkt. #2 at 2. Although Plaintiff attaches an inmate statement report, Dkt. #2 at 34, there is no indication that it is certified. Id. Accordingly, Plaintiff is directed to resubmit his IFP Request with trust account statement, certified and completed by the institution of incarceration, using the form provided by the Court within thirty (30) days of this Order, by October 4, 2021.
