As noted on the form, due to his place of incarceration, Plaintiff need not complete section 2 or provide a copy of his prison trust account, as the Court will obtain these items directly from the facility.Plaintiff must complete section 1, which includes his declaration under penalty of perjury and authorization to deduct the full filing fee from his prison trust account over time.
