ORDER signed by Magistrate Judge Gregory G. Hollows on 4/8/09 ORDERING that pltf's appl to proceed IFP is GRANTED. The Clerk is directed to issue forthwith, and the USM is directed to serve within 90 days of the date of this order, all process. The Clerk shall send pltf three USM-285 forms, one summons, a copy of the complaint, a consent form for consent to trial by a magistrate judge, and this court's status order. Pltf is directed to supply the USM, within 15 days from the date this order is filed, all info needed by the Marshall to effect service of process.
