Section 2244 for an Order Authorizing the District Court to Consider a Second or Successive Application for Relief Under 28 U.S.C. Section 2254 does not contain Petitioner's signature. If a signed copy is not filed within 14 days of this order, the case may be dismissed without further notice. Furthermore, additional documents are required for the Court to consider Petitioner's motion. Accordingly, it is hereby ORDERED that Petitioner submit the following documents within 21 days of the date of this order: (1) a completed form Section: 2244 application or, if the form application is not used, an application containing the information requested in the form; (2) the new proposed habeas petition; (3) the petition(s) filed in prior habeas action; (4) any magistrate judge's report(s) regarding the prior habeas action; and (5) the district court opinion(s) and/or order(s) regarding the prior habeas action. A copy of these documents must be served on the Lehigh County District Attorney. Petitioner must also file a certificate of service with the Clerk of the Court of Appeals demonstrating that these documents have been served.
