ORDER (Clerk) A review of the District Court docket shows that the $505.00 filing fee for Appeal No. 18-3494 was refunded to Ms. Dionne Johnson on October 17, 2019, before the above-docketed appeal was filed in the District Court and before Ms. Johnson requested that the Clerk of the District Court apply the refund to Appellant's current appeal. Given this, the Clerk of the District Court cannot use the refund to satisfy the fee in this case. Therefore, Appellant must, within fourteen (14) days of the date of this order, either pay the $505.00 filing fee to the District Court or file the following in this Court: 1) Motion for Leave to Proceed In Forma Pauperis; 2) affidavit of poverty with addendum (form enclosed); 3) authorization for withdrawl of funds (form enclosed); 4) a prison account statement for the 6 month period prior to the fililng of the notice of appeal; 5) Motion demonstrating Imminent Danger of Serious Physical Injury. Failure to either pay the filing fee or file the motions with all necessary forms within the time specified will result in the case being dismissed without further notice. See 3d Cir. L.A.R.
