Within fourteen (14) days of the date of this Order, Petitioner must provide the Court with a certified statement of his institutional account for the six months prior to this date. In addition, a certificate of service reflecting that the petition has been provided to the District Court judge must also be submitted to the Court of Appeals within fourteen (14) days of the date of this Order. If the certified account statement and proof of service are not filed with the Court, the petition may be dismissed without further notice.
