If Appellant wishes to proceed with the appeal, he must, within fourteen (14) days of the date of this order, either pay the $505 fee to the District Court or file the following: Motion for Leave to Proceed In Forma Pauperis, Affidavit of poverty with addendum (form enclosed); Authorization for withdrawal of funds (form enclosed), and Prison account statement for the 6-month period prior to the filing of the notice of appeal.
