As Kenneth and Janice are married, they may choose to file a joint affidavit of poverty provided that the joint affidavit contains both of their original signatures. Kristina must file a separate affidavit (enclosed) containing her original signature. The affidavits of poverty must be filled out completely. All questions must have a response, even if the answer is "0", "none," or "not applicable." The affidavits of poverty must be filed within 14 days of the date of this order.
