Section 1915(d). If Appellant cannot afford counsel, a motion to proceed in forma pauperis and an affidavit of poverty must be filed. Insofar as the provisions of the Prisoner Litigation Reform Act, apply to this appeal, Appellant must also file an addendum to the affidavit of poverty, a prison account statement for the 6-month period prior to the filing of the notice of appeal and an authorization for withdrawal of funds. Additional copies of the required forms are enclosed. To the extent Appellant may be seeking additional time to file those forms, the request for extension is granted.
