| Filing 5 Filed clerk order (Deputy Clerk: DA): The court has received miscellaneous physical documents and items sent to the court, including cash, some of which are addressed to Chief Judge Thomas. Along with this order, the court is returning to appellant the physical documents and items, cash, and corresponding receipts for cash sent to the court. All communications to the court should be addressed to the Clerk and not to specific judges. See 9th Cir. R. 25-2. The Court will return unfiled any miscellaneous communications that are directed to specific judges, or are not papers required or permitted to be filed in a court of appeals pursuant to the Federal Rules of Appellate Procedure. A review of the docket reflects that appellant filed a statement in response to the courts October 19, 2021 order to show cause (Docket Entry No. [ # 3 ]), but has failed to either (1) file in this court a motion to proceed in forma pauperis for this appeal, OR (2) pay to the district court $505.00 for the filing and docketing fees for this appeal AND file in this court proof that the $505.00 was paid. The court sua sponte grants appellant an extension of time to comply with the October 19, 2021 order. Within 21 days after the date of this order, appellant must pay the fees, or file a motion to proceed in forma pauperis with a completed Form 4 financial affidavit. If appellant fails to comply with this order, the appeal will be dismissed by the Clerk for failure to prosecute. See 9th Cir. R. 42-1. Briefing remains stayed pending further order of the court. The Clerk shall send a Form 4 financial affidavit to appellant along with this order. The return address on the physical items sent to the court differs from the address on the courts docket. The court will return the physical items and send a copy of this order to the return address on the envelopes: 42 Selma Avenue, Danville, VA 24541. The court will also send a copy of this order (without the physical items) to appellants address on the docket, 535 E. Yanonali St., Santa Barbara, CA 93103.
