Plaintiff is directed to provide a complete Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 Court-approved form that includes all pages. In the October 26, 2018 Order, Plaintiff was directed to provide the signed authorization to allow the agency to disburse funds from her inmate account, which is part of the Motion and Affidavit form. She has failed to do so. If Plaintiff fails to comply within the time now allowed the action will be dismissed without notice.
