MINUTE ORDER In response to the Court's December 21, 2021 Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 ), Plaintiff has submitted a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #5 ), along with a certified copy of his inmate account statement for the six months preceding his filing. Plaintiff, however, has not cured all filing deficiencies as directed. As previously ordered, Plaintiff must submit a signed authorization to calculate and, if necessary, disburse filing fee payments from his inmate trust fund account. The authorization is at p. 3 of the Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915, a copy of which was mailed to Plaintiff on December 21, 2021. Plaintiff shall submit the signed authorization form within 30 days from the date of this minute order. Plaintiff may obtain another copy of the form at www.cod.uscourts.gov.
