It is FURTHER ORDERED that Plaintiff shall obtain (with the assistance of his case manager or the facility's legal assistant) the court-approved Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form at www.cod.uscourts.gov. Plaintiff shall use the form to cure the filing deficiencies and shall notify the Court promptly if he is unable to access the necessary form.
