July 5, 2022 Opinion or Order Filing 6 MINUTE ORDER by Magistrate Judge Gordon P. Gallagher on 7/05/2022. On July 1, 2022, Mr. Zimmer filed a Prisoner's Motion and Affidavit for Leave to Proceed on Appeal Pursuant to 28 U.S.C. 1915 and Fed. R. App. 24 (ECF No. #4 ). The 1915 Motion is not on the correct form. In addition, the certified account statement submitted by Plaintiff for the six months preceding his filing does not show the balance in his account at any time. Mr. Zimmer will be afforded a final opportunity to comply with the June 13 Order. The clerk of the court shall send Mr. Zimmer a copy of the court-approved Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form, which Plaintiff shall use to cure the filing deficiency within 30 days from the date of this minute order. Plaintiff must also submit, within 30 days, a certified copy of his inmate trust fund account statement showing the balance in his account each month for the past six months.
