Filing 9 MINUTE ORDER by Magistrate Judge Gordon P. Gallagher on 09/13/2022. After initiating this action on July 18, 2022 (ECF No. #1 ), Plaintiff was directed on July 20, 2022, to cure filing deficiencies by submitting a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #3 ). He has thus far failed to do so. In a minute order entered on August 10, 2022 (ECF No. #6 ), the Court again directed Plaintiff to submit to the Court, within 30 days, a fully completed Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. The Clerk of the Court mailed a copy of the court-approved form and applicable instructions to Plaintiff. On September 8, 2022, Plaintiff filed a Prisoner's Motion and Affidavit for Leave on Appeal Pursuant to 28 U.S.C. 1915 and Fed. R. App. P. 24 (ECF No. #8 ), which is not on the correct form for this civil rights action. Plaintiff is instructed that he must complete and submit all pages of the form on or before October 13, 2022, or this action will be dismissed without prejudice and without further notice.
