Plaintiff's Second Amended Complaint (ECF No. 13) is deficient because Plaintiff has not used the proper Prisoner Complaint form as directed and required by the Court's local rules. See D.C.COLO.LCivR 5.1(c). Plaintiff will be given another opportunity to file an amended complaint on the proper form and must do so on or before October 14, 2022. The Clerk of Court is directed to mail to Plaintiff, together with a copy of this Minute Order, a blank copy of the District of Colorado Prisoner Complaint form.
