August 22, 2022 Filing 5 AMENDED ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES AND ORDER TO SHOW CAUSE. It is ORDERED that within thirty (30) days from the date of this Order, Plaintiff shall submit a Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 or pay the $402.00 filing fee. Plaintiff must also submit a Prisoner Complaint. It is FURTHER ORDERED that if Plaintiff submits a 1915 Motion and Affidavit, he must also show cause, in writing, within thirty (30) days from the date of this Order, why he should not be denied leave to proceed in forma pauperis because he is subject to the filing restriction in 28 U.S.C. 1915(g). It is FURTHER ORDERED that, if Plaintiff files a 1915 Motion and Affidavit, but fails to show cause as directed above within the time allowed, he will be denied leave to proceed in forma pauperis. It is FURTHER ORDERED that Mr. Zimmer shall obtain copies of the forms for filing (1) a Prisoner Complaint, and (2) a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (with the assistance of his case manager or the facility's legal assistant), at www.cod.uscourts.gov. It is FURTHER ORDERED that, if Mr. Zimmer fails to cure all designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice.
