It is ORDERED that Mr. Murphy cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that Mr. Murphy files in response to this order must be labeled with the civil action number identified on this order. It is FURTHER ORDERED that, with the assistance of his case manager or the facilitys legal assistant, Mr. Murphy shall obtain and utilize the appropriate court approved form for filing a Prisoner Complaint and a Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. He may pay the $402 filing fee instead of submitting a Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. The court-approved forms and applicable instructions are available on the Courts website at: http://www.cod.uscourts.gov. It is FURTHER ORDERED that if Mr. Murphy fails to cure the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice.
