Failure to comply may result in dismissal of this action without prejudice and without further notice. The clerk of the court is directed to mail to Plaintiff a copy of the Prisoner Complaint form, along with the applicable instructions. If Plaintiff is unable to obtain a certified copy of his account statement for the six months preceding his filing (or for the period of his detention if less than six months), he shall submit to the Court, within 30 days from the date of this minute order, a signed detailed affidavit explaining (1) the facility's procedure for obtaining a certified account statement; (2) his efforts to obtain a certified account statement by complying with that procedure; and (3) an explanation of how detention center officials (who must be identified) have refused to provide him with a certified account statement.
