Minute ORDER by Magistrate Judge Scott T. Varholak on 5 September 2023. The Motion to Amend (ECF No. #8 ) is GRANTED to the extent that the Court has ordered Plaintiff to cure his pleading (see ECF No. #7 ), and Plaintiff may include any amendments in his cured pleading. The Motion for Reconsideration (ECF No. #9 ) is construed as both a request for the Court-approved forms and an extension of time to cure deficiencies. The motion is GRANTED to the extent that the Plaintiff shall have up to and including October 6, 2023, to cure the deficiencies. Additionally, the Clerk of Court is directed to mail to Plaintiff, together with a copy of this Minute Order, blank copies of the court-approved Prisoner Complaint form and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form, along with the applicable instructions.
