If Plaintiff is unable to submit a certified copy of his inmate account statement by the deadline, he must provide a sworn declaration to the Court, by the filing deadline, stating the names of the prison official(s) from whom he requested and was denied a copy of his account statement, the dates of the request(s) and denial, the stated reasons therefor, and whether he has filed a grievance regarding the denial.
