MINUTE ORDER The Clerk of Court is directed to change Plaintiffs information in the docket to reflect that Plaintiff is currently confined at the DRDC and his prisoner identification number is 199240. Further, the Clerk of Court is directed to mail to Plaintiff, at the DRDC, a copy of this Minute Order and a copy of the January 16, 2024 Order Directing Plaintiff to Cure Deficiencies (ECF No. #4 ). Plaintiff will have thirty (30) days from the date of this Minute Order to cure the deficiencies as directed. If Plaintiff fails to cure the deficiencies within the time allowed, theaction will be dismissed without further notice. Finally, Plaintiff is notified that, pursuant to this Courts local rules, he must inform the Court of a change of address within five (5) days after the change. ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES Plaintiff shall cure the deficiencies designated above within 30 days from the date of this Order. Any papers that Plaintiff files in response to this Order must include the civil action number on this Order. Plaintiff shall either submit a complete court-approved Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form or, in the alternative, pay the $405.00 filing and administrative fees. Plaintiff may obtain copies of court-approved forms (with the assistance of his case manager or the facility's legal assistant), along with the applicable instructions, at www.cod.uscourts.gov. If Plaintiff fails to cure the designated deficiencies within 30 days from the date of this Order the action will be dismissed without further notice. Further plaintiff states that if is mail is returned to the Court, the Court should resend it to the DOC. (ECF No. 1 at 2). Plaintiff is notified that, pursuant to this Court's local rules, it is his responsibility to notify the Court of a change of address within five (5) days after the change. See D.C.COLO.LCivR 5.1(c) ("Notice of change of name, mailing address, or telephone number of an unrepresented prisoner or party shall be filed not later than five days after the change.").
