MINUTE ORDER On February 26, 2024, the Court issued an Order Directing Plaintiff to Cure Deficiencies (ECF No. 3). In response, Plaintiff has submitted to the Court a 23-page Prisoner Complaint (ECF No. 6), a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. 7), and a 6-page partial Prisoner Complaint (ECF No. 8). These filings do not cure all designated deficiencies. All of Plaintiff's claims and allegations must appear in one, single amended Prisoner Complaint that he files in response to this order. Plaintiff is instructed that he must complete all sections and submit all pages of his amended Prisoner Complaint using the court-approved formon or before April 25, 2024. Plaintiff remains obligated to submit on or before April 25, 2024, a certified copy of his inmate trust account statement showing the account activity for the past 6 months. If Plaintiff cannot obtain a certified copy, he shall submit to the Court a detailed affidavit explaining (1) the facility's procedure for obtaining the inmate account statement; (2) his efforts to obtain the account statement by complying with that procedure; and (3) an explanation of how prison officials (who must be identified) have refused to provide him with the statement. Vague or conclusory allegations will not suffice. Plaintiff is warned that if he fails to comply within the time allowed, the action may be dismissed without further notice. ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES It is ORDERED that Plaintiff cure the deficiencies designated above ithin thirty (30) days from the date of this order . Plaintiff shall obtain and utilize (with the assistance of a case manager or the facility's legal assistant) the court-approved form 1) Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915, and 2) Prisoner Complaint along with the applicable instructions, available at www.cod.uscourts.gov. If Plaintiff fails to cure the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice.
