Filing 6 SECOND ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES. It is ORDERED that Plaintiff cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that he files in response to this order must be labeled with the civil action number identified on this order. It is FURTHER ORDERED that Plaintiff shall obtain and use the current, court-approved forms: (1) Prisoner Complaint and (2) Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form, both of which are available (with the assistance of a case manager or the facility's legal assistant), along with the applicable instructions, at www.cod.uscourts.gov. Plaintiff may pay the $405.00 filing and administration fee in lieu of filing a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. It is FUTHER ORDERED that the Clerk of Court is directed to mail one copy of the Prisoner Complaint and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 forms to Plaintiff for his use. It isFURTHER ORDERED that if Plaintiff fails to cure the designated deficiencies within thirty (30) days from the date of this order, the action may be dismissed without further notice.
