MINUTE ORDER by Magistrate Judge Susan Prose on 5/7/2024. In response to the Court's March 26, 2024 Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 ), Plaintiff submitted two Applications to Proceed in District Court Without Prepaying Fees or Costs (Long Form) (ECF Nos. #4 and #5 ) and an account statement (ECF No. #6 ). The Applications are denied as improperly filed because Plaintiff is an inmate at the Denver County Jail, and therefore, must use the forms for prisoners. In addition, Plaintiff has not filed her claims on the court's Prisoner Complaint form. Plaintiff will be given one final opportunity to cure all filing deficiencies in this action. To comply, she must submit a Prisoner Complaint as well as a certified copy of her inmate account statement for the preceding six-month period (i.e., October 2023 present) as well as a signed authorization to calculate and disburse filing fee payments. If Plaintiff fails to submit these documents on or before June 6, 2024, then this action will be dismissed without further notice.(dgumb, )

ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES by Magistrate Judge Susan Prose on 26 March 2024. ORDERED that Plaintiff cure the deficiencies designated above within thirty days from the date of this order if she wishes to pursue claims in this action. Any papers that Plaintiff files in response to this order must include the civil action number on this order.
