ORDER TO CURE DEFICIENCIES It is ORDERED that Plaintiff cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that Plaintiff files in response to this order must include the civil action number on this order. It is FURTHER ORDERED that Plaintiff shall obtain the Court-approved forms for filing a Complaint and (if he wishes to proceed to proceed in forma pauperis) an Application to Proceed in District Court Without Prepaying Fees or Costs (Long Form), along with the applicable instructions, at www.cod.uscourts.gov. Plaintiff shall use the court-approved forms when curing the deficiencies. It is FURTHER ORDERED that Plaintiffs "Motion to Amend" (ECF No. #2 ) is denied. Plaintiff shall include any new claims and/or factual allegations when he cures the deficiencies by filing his pleading on the court-approved form. It is FURTHER ORDERED that if Plaintiff fails to cure the designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice.
