July 1, 2024 Opinion or Order Filing 4 ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES Plaintiff shall cure the deficiencies designated above within thirty (30) days from the date of this Order. Plaintiff shall either submit a Court-approved Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 form along with a certified copy of prisoner's trust fund statement for the 6 month period immediately preceding this filing (or for the period time Plaintiff has been in custody if less than six months) or, in the alternative, pay the $405.00 filing fee. If Plaintiff fails to cure the designated deficiencies within thirty (30) days from the date of this Order the action will be dismissed without further notice. The Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. 2) is denied without prejudice with leave to refile with a certified copy of prisoner's trust fund statement for the 6 month period immediately preceding this filing (or for the period time Plaintiff has been in custody if less than six months).
