MINUTE ORDER by Magistrate Judge Richard Thomas Gurley on 8/28/2024. In response to the Court's Order Directing Plaintiff to Cure Deficiencies (ECF No. #6 ), Plaintiff has submitted a Prisoner Complaint (ECF No. #7 ). Plaintiff has failed to cure all deficiencies because he has not submitted a Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. Plaintiff will be given one more opportunity to cure this remaining deficiency and must do so on or before September 27, 2024. Plaintiff is advised that the action will be dismissed without further notice if he fails to cure the remaining deficiency as previously directed within the time allowed. (dgumb, )

ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES. It is ORDERED that Plaintiff cure the deficiencies designated above within thirty (30) days from the date of this order. Any papers that Plaintiff files in response to this order must include the civil action number on this order. Plaintiff shall obtain (with the assistance of a case manager or the facility's legal assistant) and then utilize the court-approved forms: (1) Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915, and (2) Prisoner Complaint. The forms along with the applicable instructions are available at www.cod.uscourts.gov. The Court also directs the clerk of the court to send Plaintiff a copy of the court-approved forms along with applicable instructions. If Plaintiff fails to cure all designated deficiencies within thirty (30) days from the date of this order, the action will be dismissed without further notice. The dismissal shall be without prejudice, by Magistrate Judge Richard Thomas Gurley on 7/29/2024. (Forms sent as ordered) (dgumb, )
