MINUTE ORDER. In response to Court's order to cure filing deficiencies (ECF No. #4 ), Plaintiff has submitted a Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #6 ) and a handwritten document (ECF No. #7 ). But these filings do not cure the identified filing deficiencies. To proceed with this action, Plaintiff must file (1) a fully completed form Prisoner Complaint, and (2) he must either pay the $405.00 filing fee or file a Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915. Thus, the Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #6 ) is denied without prejudice to being re-filed on the correct form. Plaintiff is granted an extension to cure these outstanding filing deficiencies, making the new deadline February 26, 2025. Plaintiff is reminded that this action will be dismissed without further notice if the filing deficiencies described above are not cured within the time allowed. By Magistrate Judge Richard T.
