May 15, 2025 Opinion or Order Filing 8 MINUTE ORDER by Magistrate Judge Richard T. Gurley on 5/15/2025. In response to the Court's March 7, 2025 Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 ), Plaintiff filed a Prisoner Complaint (ECF No. #4 ) and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #5 ). However, because Plaintiff had not submitted a certified copy of his inmate account statement showing activity for the six months preceding his filing, the Court issued a minute order informing Plaintiff that he remained obligated to submit his account statement on or before May 14, 2025. (See ECF No. #6 ). In response, Plaintiff filed a "Supplement in Support of IFP" (ECF No. #7 ), asserting that his case manager gave him "6-month account statements before [his] case and point U.S. District Court of Colorado case no. 1:24-cv-01542-LTB-RTG." Plaintiff will be given one final opportunity to cure the remaining filing deficiency in this action. To be clear, Plaintiff must file on or before June 13, 2025, a certified copy of his inmate account statement for the preceding six-month period. If he fails to do so, this action will be dismissed without further notice. (dgumb, )

On March 7, 2025, the Court issued an Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 ). In response, Plaintiff has submitted, on April 11, 2025, a Prisoner Complaint (ECF No. #4 ) and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #5 ). March 7, 2025 Opinion or Order Filing 3 ORDER DIRECTING PLAINTIFF TO CURE DEFICIENCIES. Plaintiff shall obtain (with the assistance of a case manager or the facility's legal assistant) and utilize the court-approved forms: (1) Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915, and (2) Prisoner Complaint. The forms along with the applicable instructions are available at www.cod.uscourts.gov. The Court also directs the clerk of the court to send Plaintiff a copy of the court-approved forms along with applicable instructions. If Mr. Hale El fails to cure the designated deficiencies as directed within thirty (30) days from the date of this Order, the action will be dismissed without further notice.
