MINUTE ORDER In response to the Courts Order Directing Plaintiff to Cure Deficiencies (ECF No. #3 ), Plaintiff has submitted a Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 in a Habeas Corpus Action (ECF No. #4 (emphasis added)). But this is not a habeas corpus action, so the motion does not cure the deficiencies as ordered. Plaintiff has failed to submit the District of Colorados courtapproved Prisoner Complaint and Prisoners Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 as ordered. Thus, the Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 in a Habeas Corpus Action (ECF No. #4 ) is denied without prejudice to being re-filed on the correct court-approved form. To proceed with this action, Plaintiff must first cure the deficiencies as ordered by the Court. Plaintiff shall have until November 14, 2025, to cure the deficiencies. Plaintiff is reminded that this action will be dismissed without further notice if the deficiencies are not cured within the time allowed. The Clerks Office is directed to mail to Plaintiff, together with a copy of this Minute Order, a copy of the following forms: (1) Prisoner Complaint, and (2) Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (and the applicable instructions for each form).
