MINUTE ORDER by Magistrate Judge Scott T. Varholak on 2/13/2026. In an October 23, 2025 Order (ECF No. #3 ), the Court directed Plaintiff to cure filing deficiencies within 30 days. Plaintiff has requested and been granted two extensions of time to cure the deficiencies based on his statements that prison officials have obstructed his efforts to obtain a certified copy of his inmate account statement. (See ECF Nos. #4 , #5 , #6 , #7 , & #8 ). Plaintiff will be afforded a final 30-day extension of time to submit a fully completed Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 to the Court. If Plaintiff still cannot obtain a certified inmate account statement, he must submit a detailed affidavit explaining (1) the facility's procedure for obtaining the inmate account statement; (2) his efforts to obtain the account statement by complying with that procedure; and (3) an explanation of how prison officials (who must be identified) have not provided him with the statement. Vague or conclusory allegations will not suffice. Plaintiff also should submit copies of any grievances or kites filed concerning attempts to obtain a certified copy of the inmate account statement. ORDER OF RECUSAL. Title 28 U.S.C. 455(b) requires that a judge disqualify himself in certain circumstances in which his impartiality might be reasonably questioned. Guided by the considerations addressed above, I conclude that it is prudent for me to recuse myself from further service in the matter.
