MINUTE ORDER. In response to the Court's November 19, 2025 Order Directing Plaintiff to Cure Deficiencies (ECF No. #4 ) and December 22, 2025 Minute Order (ECF No. #8 ), Plaintiff filed a Prisoner Complaint (ECF No. #9 ) and Prisoner's Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #10 ). Both filings are on the court approved forms. However, instead of attaching a copy of his inmate trust fund account statement from the Denver County Jail (which is Plaintiffs current address of record), he submitted a personal account ledger from the Department of Human Services from March 1, 2025 to September 22, 2025, when he was housed at the Colorado MentalHealth Hospital in Pueblo, Colorado. To proceed with this action, Plaintiff also must submit a certified copy of his inmate account statement from the Denver County Jail. Plaintiff will be given one final opportunity to cure this remaining deficiency. To be clear, Plaintiff must file on or before February 23, 2026, a certified copy of his inmate account statement from the Denver County Jail. If he fails to do so, this action may be dismissed without further notice. It is further ordered that the Motion and Affidavit for Leave to Proceed Pursuant to 28 U.S.C. 1915 (ECF No. #2 ) is denied as improperly filed and the letter requesting copies and an extension of time filed on December 19, 2025 (ECF No. #7 ) is denied as moot.
