Filing 11 ORDER TO SHOW CAUSE why this matter should not be dismissed for failure to file an amended complaint or other supplemental information as outlined in the Court's June 13, 2019, Order (Doc. #8).The self-represented plaintiff filed this action on a form Complaint for Review of Social Security Administration Decision. (Doc. #1 ). As indicated in the Court's June 13, 2019, Order, plaintiff had not fully completed the form Complaint, including the date on which the Appeals Council issued its decision. See Doc. #8. Without that information, the Court is unable to ascertain whether plaintiff has fully exhausted his administrative remedies and/or timely filed this action.On June 13, 2019, the Court ordered that by July 8, 2019, plaintiff file a completed amended complaint, or alternatively, any supplemental information (such as a copy of the Appeals Council decision), which would cure the deficiency identified in the June 13, 2019, Order. See id. A copy of that Order was mailed to plaintiff at his address of record on June 13, 2019. To date, plaintiff has failed to file either an amended complaint or any supplemental information. Accordingly, plaintiff is hereby ORDERED TO SHOW CAUSE why this matter should not be dismissed for his failure to file an amended complaint or other supplemental information as outlined in the Court's June 13, 2019, Order (Doc. #8). Plaintiff's show cause response is due on or before August 12, 2019. Should plaintiff fail to respond to this Order or to comply with the Court's June 13, 2019, Order by August 12, 2019, then his case may be dismissed.The Clerk of the Court is directed to mail a copy of this Order, and an additional copy of the Court's June 13, 2019, Order (Doc. #8), to plaintiff at his address of record. Filing 7 ORDER granting #2 Motion for Leave to Proceed in forma pauperis. Plaintiff has filed a motion seeking to proceed without payment of fees and costs, which motion includes a sworn statement as to plaintiff's current financial circumstances. See Doc. #2 . Plaintiff asserts that he is unable to afford to pay fees and costs, having no income or assets. See generally id. at 3-4. Plaintiff also states that he receives $219 per month in benefits. See id. at 3. At this stage, such allegations are sufficient to establish that plaintiff is unable to pay the ordinary filing fees required by the Court. 28 U.S.C. 1915(a)(1).
