Filing 6 NOTICE OF INSUFFICIENCY. In a complaint dated February 8, 2022, the plaintiff sued several prison officials, substantially alleging that they were responsible for his contracting COVID-19. The Clerk of the Court docketed his complaint under case number 22-cv-00559. The plaintiff also moved for leave to proceed in forma pauperis in that case; in other words, he asked for permission to begin his case without prepaying the $402 filing fee. He failed to submit a complete in forma pauperis application, however, and he also failed to submit a certified copy of his inmate trust account statement as required by the relevant statute, 28 U.S.C. 1915. Accordingly, on April 21, 2022 the Court ordered him to submit a complete application and trust account statement by May 12, 2022.On May 9, 2022, the Court received a second complaint from the same plaintiff, also substantially alleging that prison officials were responsible for his contracting COVID-19. The Clerk of the Court docketed this second complaint under case number 22-cv-00648. It is not clear to the Court that the plaintiff really intended to start a second case. If he did, he must either pay the full filing fee or demonstrate that he is entitled to in forma pauperis status. His in forma pauperis motion in his second case fails to make that demonstration, for the same reasons as in his first case; namely, that he did not fill out the five-page application completely, and he did not attach a certified copy of his inmate trust account statement.If the plaintiff wishes to pursue this second case, he is ordered to file (a) a complete Prisoner Application to Proceed In Forma Pauperis in a Civil Rights Action, including page five; and (b) a certified copy of his inmate trust account statement for the period November 2, 2021 to May 2, 2022.
