Filing 7 NOTICE OF INSUFFICIENCY. A plaintiff may commence a federal lawsuit without pre-payment of the filing fee if he shows his inability to pay such fees in accordance with federal statute 28 U.S.C. 1915. To make this showing, the plaintiff must, among other things, submit an affidavit and a certified copy of a trust fund account statement for the 6-month period immediately preceding the filing of the complaint. 28 U.S.C. 1915(a)(1)-(2). Here, the plaintiff submitted an unsigned affidavit and a trust fund account statement that does not reflect balances of the 6-month period preceding the complaint. The plaintiff is directed to file with the Clerk of the Court a signed affidavit, as well as a certified copy of his inmate trust account statement covering the period from at least December 5, 2022, to June 5, 2023.
