August 15, 2023 Filing 6 NOTICE OF INSUFFICIENCY. The plaintiff has filed a motion for leave to proceed in forma pauperis; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. 1915(a)(1), permits him to do so if he files an affidavit demonstrating that he is unable to pay the fee. The filing fee for a habeas petition is five dollars. In this case, the plaintiff has submitted a signed financial affidavit, but the form he used does not contain a separate section for the balance of a prisoner's inmate trust account. To assist inmate plaintiffs in moving for in forma pauperis status, the District of Connecticut provides them with a six-page form to be filled out. Without knowing the balance of the plaintiff's inmate trust account, the Court cannot determine whether he is able to pay.The Clerk of the Court is respectfully directed to mail the plaintiff a copy of the application form at his address of record. The plaintiff is advised that he need not complete pages four and five of the form, since he has not filed a civil rights complaint. The plaintiff is also advised, however, that he must carefully and completely fill out the rest of the form, including accurately stating the balance of his inmate trust fund account statement. The plaintiff is thus directed to complete the form and return it to the Clerk of the Court by September 5, 2023. If he does not do so, his case may be dismissed. Signed by Judge Thomas O.
