ORDER: Plaintiff has filed this action pursuant to 42 U.S.C. 1983 without prepayment of the filing fee and has submitted for consideration an unsigned application to proceed in forma pauperis. Pursuant to 28 U.S.C. 1915(b), Plaintiff shall be assessed the filing fee of $350.00. The Clerk of Court shall provide Plaintiff with a copy of the as-filed, unsignedapplication to proceed in district court without prepaying fees or costs. (D.I. #6 ). Plaintiff shall sign and return the application within thirty (30) days. Failure of Plaintiff to return the signed application within thirty (30) days from the date of this Order shall result in dismissal of this action without prejudice.
