TEXT ORDER. Plaintiff filed an improper disclosure form. (Doc. 17). Local Rule 3.03 requires that each party file a disclosure form with their first appearance, and we further emphasize this requirement in our Civil Action Order. (Doc. #5 at 2). We even entered a text order (Doc. 16) that reminded plaintiff of this requirement, and included a URL link to the proper form. As plaintiff has a workable email and informed us in a separate case that he monitors it daily (2:25-cv-536-JES-NPM, Doc. 36 at 1), we infer that plaintiff should have been able to use the link we provided but deliberately chose not to. By December 15, 2025, plaintiff must either file a proper disclosure form, or show cause why his case should not be dismissed for failure to comply with court orders.
