Filing 22
TEXT ORDER. Plaintiff filed an improper disclosure form. (Doc. 20). Local Rule 3.03 requires that each party file a disclosure form with their first appearance, and we further emphasize this requirement in our Civil Action Order. (Doc. #4 at 2). We even entered a text order (Doc. 19) that reminded plaintiff of this requirement, and included a URL link to the proper form. As plaintiff has a workable email and stated he has a PACER account and the ability to comply with the technical requirements of CM/ECF (Doc. #7 at 1), we infer that plaintiff should have been able to use the link we provided but deliberately chose not to. By December 15, 2025, plaintiff must either file a proper disclosure form, or show cause why his case should not be dismissed for failure to comply with court orders. The clerk is DIRECTED to delete the PDF file (Doc. 20) from the CM/ECF docket. Signed by Magistrate Judge Nicholas P. Mizell on 12/1/2025. (RCD)

TEXT ORDER. Local Rule 3.03 requires that, with the first appearance, each party file a disclosure statement using the standard form from the clerk or the court's website. Plaintiff failed to do so.
