April 7, 2026 Opinion or Order Filing 3 ENDORSED ORDER: Plaintiff filed a four-page, unsigned document #1 in which he appears to allege he is being denied access to sufficient mental health treatment and the grievance process. The document is not a proper complaint, does not include all the information required by the court-approved civil rights complaint form, and cannot commence a civil action. See Fed. R. Civ. P. 3 ("A civil action is commenced by filing a complaint with the court."); see also Local Rule 6.04(a) ("A pro se person in custody must use the standard form[.]"). Additionally, Plaintiff failed to sign the document as required by the Federal Rules of Civil Procedure. See Fed. R. Civ. P. 11(a) ("Every pleading... must be signed... by a party personally if the party is unrepresented."). Accordingly, the Clerk is directed to close this case and send Plaintiff a civil rights complaint form. Plaintiff may commence a new action by using the form. Signed by Judge Wendy W.
