ENDORSED ORDER. Plaintiff, Carlos Ruiz, an inmate of the Florida penal system, filed a pro se document #1 titled, "Petitioner Accuse and Request Protection Pursuant to Fla. 20.315(1)(d)," in which he complains that his cellmate attacked him, Officer Alston and other officers beat him to unconsciousness, Officer Alston destroyed his personal property, and he incurred a "bogus disciplinary report." Ruiz's filing is not a proper complaint, does not include all of the information required by the court-approved civil rights complaint form, and cannot commence a civil action. See Fed. R. Civ. P. 3 ("A civil action is commenced by filing a complaint with the court."); see also M.D. Fla. R. 6.04(a) ("A pro se person in custody must use the standard form[.]").
